EVERYTHING YOU NEED TO KNOW AS A NEW SCOUT

It can be overwhelming to enter a new troop and be confronted with a plethora of forms to be filled out, notarized, and returned. Hopefully, this page will help you sort through the heap of necessary paperwork. All forms may be returned to the New Scout Coordinator or the Committee Chair.

 1. The first thing you will need to fill out as a parent with a new scout is his youth application. This is the form with the carbon copies that contains all his personal information such as name, age, address, etc. This form is completed and returned with your son's registration fee, and a fee for Boy's Life Magazine, if you choose to subscribe. If your son filled one of these out in his pack, he'll still need to fill out one for the troop, as it is a separate entity. The fees are also separate from any he may have paid with the pack. These forms are available at troop meetings or at the Scout Office.

2. The second form may be the adult application. It is recommended that at least one parent or guardian sign up to be involved with the troop. You have the choice of becoming a committee member or an assistant scoutmaster. As a committee member, you may sit in on boards of review for the boys and have a vote at committee meetings. As an assistant scoutmater you may sign off on boys' requirements in their books. There is a need for both positions, and holding one position or another does not restrict what you are allowed to do as far as activities with the troop. These forms are also available at troop meetings or at the Scout Office.

3. Troop By-Laws Acknowledgement form. The by-laws may be read online at http://troop400.com/bylaws.rtf or by clicking on the Operating Guidelines link on the website. Our Operating Guidelines, also known as our by-laws, contains information on how the troop is run, and contains the rules and requirements set forth by the troop committee. The by-laws need to be read by all parents/guardians, and also by the scout. The parents and the scout then sign the acknowledgement form and return it to the troop. You may download a copy of the acknowledgement form here.

4 The. Troop 400 Parental Release Form.can be downloaded here. This form needs to be signed and notarized when a boy joins the troop. This troop is an agreement between parents and the troop concerning discipline, medical emergencies, and finances.

5. Physical/Medical form. Each boy must keep on file a current copy of their physical and a copy of their insurance card. The physical form can be downloaded here. This form must be completed by a doctor or nurse practitioner and must be kept current every year. Also, all adults who will be camping or participating in troop events need to have the same form on file for themselves.

ADDITIONAL FORMS

1. A Troop Resource Survey can be downloaded here. This form simply gives adults the chance to share their talents with the troop by taking a survey of their resources, talents, and abilities

2. A Parental Consent Form is required for every trip or activity in which the scout participates. This form can be downloaded here. The front and back of this sheet must be filled out every time with updated medical information on the back. This form will need to be notarized if the troop is traveling out of council, out of state, or more than 500 miles from home.

3. Our scoutmater, Moni Boling, has written a welcome letter for boys joining the troop, outlining some of the policies and procedures of the troop. You can download the letter here.

4. The council has put together a list of merit badge counselors for our district, which can be downloaded here (xls file). Boys wishing to begin a new merit badge need to contact the merit badge counselor in our troop for that particular badge. Merit badge classes are offered as there is interest and time to include them in the schedule. Often, classes are taught before regular troop meetings. Here is a handy link from national that explains merit badges in more detail.

Additional forms may be found on our website under the Scout Forms link.
